Matos LeBron Consulting is a small, private practice offering direct -pay case management services. We do not participate in insurance billing at this time; however, client may request a Superbill to submit to their insurance company for potential out of net-work reimbursement.
Our case management services are designed for individuals and families who need personalized, hands-on support navigating healthcare, social services, and daily life needs without long waitlist or services limitations.
All client begin with a comprehensive initial evaluation to assess needs, identify barriers, and create an individualized case management plan.

This includes a 90-minute intake assessment, care plan development, one in-home visit, one round trip transportation services, and full access to case management support during the first month.
We offer flexible monthly case management memberships designed to meet varying levels of support needs. All memberships include personalized care coordination, advocacy, and ongoing support, with services tailored to each client’s situation.
This membership is ideal for clients who need ongoing coordination and support but minimal in-person services.
Includes care coordination and service planning, appointment scheduling and reminders, assistance with community resources and benefits, provider communication and advocacy, monthly check-ins, and one round-trip transportation service per month for medical appointments, pharmacy pickup, or essential services.
Designed for clients who require more frequent hands-on support and follow-up.
Includes everything in the Basic Membership, plus one in-home visit per month, increased provider coordination, ongoing medication support and pharmacy coordination (including assistance with medication delivery when available), family communication and updates as requested, and priority scheduling.
Best suited for clients with complex needs or families seeking a higher level of involvement and advocacy.
Includes everything in the Premium Membership, plus additional in-home support as needed, enhanced advocacy during medical or service-related appointments, extended care coordination, caregiver support, and customized service planning based on client needs.
Additional transportation services beyond the monthly included round trip may be added for a flat monthly fee or per-ride rate, depending on client needs. Transportation availability is coordinated as part of the case management plan.
As part of case management services, we assist clients with medication coordination, including pharmacy communication and identifying insurance-covered delivery options when available, to reduce the need for in-person pickup and improve medication adherence.
